- ✓ Same-day bookings are accepted — call (516) 754-7777 as soon as discharge is confirmed
- ✓ Have someone at the hospital notify us when the discharge order is signed
- ✓ Gather all personal belongings, prescriptions, and discharge paperwork before pickup
- ✓ Let us know about any new mobility equipment from the hospital stay
- ✓ Confirm your home address including any access notes such as elevator or gate code
